What is subacromial impingement/bursitis. The space above the glenohumeral (ball and socket) joint and under the acromion (bony tip of the shoulder) is the subacromial space. This area has a fluid filled sac in place, which can become painful and inflamed over time with repetitive movements of the arm. Subacromial impingement refers to the pain, weakness and loss of movement at the shoulder due to irritation of the subacromial structures i.e. supraspinatus tendon, and bursa as they pass through Subacromial bursitis is a condition caused by inflammation of the bursa that separates the superior surface of the supraspinatus tendon (one of the four tendons of the rotator cuff) from the overlying coraco-acromial ligament, acromion, and coracoid (the acromial arch) and from the deep surface of the deltoid muscle. With subacromial impingement, the supraspinatus tendon of the rotator cuff is involved because it slips under the acromion and attaches into the greater tubercle, a bony bump on the humerus.

Charles Neer, in 1972, described the presence of a "proliferative spur and ridge" on the undersurface of the acromion, which needs to be removed to improve the symptoms (acromioplasty).

Subacromial impingement occurs when the tendons of the shoulder joint (or rotator cuff) are injured as they glide against the bones and ligaments of the shoulder. More specifically, the rotator cuff tendons can rub against the acromion process during overhead activities which can lead to pain and degeneration of the tendons.
